You may register by E-mail, Fax, in person or register by downloading the registration form that is on our website.  A registration form is available below.

The following pieces of information are required at the time of registration for each child:

• A completed Registration form
• Copy of Birth Certificate 
• Copy of Child's Public Health Immunization Card
• Payment of Fees (registration and down payments) as required.
• Signed Contractual Agreement
• A picture of your Child (optional)
